Day Program Case Manager

 

 

Summary:

The Day Program Case Manager reports directly to the Day Program Supervisor and is responsible for coordination and monitoring the assessment, treatment planning process and follow up for each consumer, serves as an advocate and liaison for and on behalf of consumers benefits. Formulates and updates reports on educational and treatment programs for all consumers. Prepare and assist staff on status of clients' progress so they may increase their knowkedge and ability to ultimately give optimum client care.  

 

Essential Functions:

  • Participate in client assessment, planning and evaluation process.
  • Participate in the development of the individual program plan.
  • Provide reports on client progress as outlined in the individual program plan.
  • Maintain/update/file client’s information and progress.
  • Implement program curricula.
  • Assist staff in delivering individual and group learning experience so each client is obtaining his/her IPP objectives.
  • Communicate on a regular basis with Directors/Managers/Regional Center/Family Care/Parents/Staff as needed.
  • Promote confidentiality concerning client/family care issues.
  • Work in conjunction with the Behavior Consultant/Analyst as needed.
  • Evaluates, facilitates and monitors services and clients progress in relationship.
  • Coordinates and chair the interdisciplinary treatment planning and meeting.
  • Evaluate, facilitate and monitor services and clients' progress in relationship to established goals and objectives.
  • Promote a clean, safe and healthy environment.
  • Report special incidents according to applicable laws and regulations.
  • Attend team meetings or other training as part of continuation of education or any agency –wide training/in-services programs. ....
  • Attend and participate in clients' quarterly/semi-annual/annual meetings. 
  • Advocate for clients to resolve crisis.
  • Collaborate with other professionals to evaluate clients' medical or physical condition and to assess client needs.
  • Monitor, evaluate, and record client progress according to measurable goals described in treatment and care plan.
  • Identify environmental impediments to client or patient progress through interviews and review of patient records.
  • Perform other related duties and assignments as required by management.

 

Qualifications: 

  • Knowledge of human behavior and performance; individual differences in ability, personality, and interests; learning and motivation; psychological research methods; and the assessment and treatment of behavioral and affective disorders.
  • Knowledge of principles, methods, and procedures for diagnosis, treatment, and rehabilitation of physical and mental dysfunctions, and for career counseling and guidance.
  • knowledge of principles and processes for providing customer and personal services.  This includes customer needs assessment, meeting quality standards for services, and evaluation of customer satisfaction.
  • Knowledge of the information and techniques needed to diagnose and treat human injuries, diseases, and deformaities.  This included symptoms, treatment alternatives, drug properties and interactions, and preventive health-care measures.
  • Knowledge of principles and methods and curriculum and training design, teaching and instruction for individuals and the measurement of training effects.
  • Knowledge of laws, legal codes, court procedures, precedents, government regulations, executive orders, agency rules, and the democratic political process.
  • All offers are contingent upon passing a background check, physical and a TB test or chest xray.
  • Must be CPR/FA, CPI Certified.
  • Must have A.A. Degree (B.A. Degree preferred).
