SLS (Supported Living Services) Staff

POSITION SUMMARY:
|
|
Coordinate and monitor the assessment, treatment planning process and follow up for each consumer, serve as an advocate and liaison for and on behalf of consumers benefits. Formulate and update reports on educational and treatment programs for all consumers. Prepare report of clients’ progress so they may increase their knowledge and ability to ultimately give optimum client care.

ESSENTIAL FUNCTIONS
|

Participate in client assessment, planning and evaluation process.
-
Participate in the development of the individual program plan.
-
Provide reports on client progress as outlined in the individual program plan.
-
Maintain/update/file date regarding client’s information and progress.
-
Implement program curricula.
-
Deliver individual and group learning experience so each client served obtains his/her IPP objectives.
-
Communicate on a regular basis with the Director/ Manager/Regional Center, Family Care/Parents as needed.
-
Promote confidentiality concerning client/family care issues.
-
Work in conjunction with the Behavior Consultant/Analyst as needed.
-
Evaluate, facilitate and monitor services and clients progress in relationship

Establish and maintain an effective liaison between and Home Providers/Supervisors and home staff on a regular basis.
-
Evaluate, facilitate and monitor services and clients progress in relationship to established goals and objectives
-
Maintain a clean, safe and healthy environment
-
Report special incidents according to applicable laws and regulations.
-
Attend team meetings or other training as part of continuation of education.
-
Maintain and promote a positive attitude toward clients and their treatment programs.
-
Monitor clients’ performance in activities, providing encouragement.
-
Instruct, or assist in instructing, clients and families in home programs, basic living skills, or the care and use of adaptive equipment.
-
Implement, or assist occupational therapists with implementing, treatment plans designed to help clients function independently.
-
Evaluate the daily living skills or capacities of physically, developmentally disabled clients.
-
Aid clients in dressing and grooming themselves.
-
Report to supervisors, verbally or in writing, on patients' progress, attitudes, and behavior.
-
Perform other related duties and assignments as required by management.

QUALIFICATIONS:
|

Knowledge of human behavior and performance; individual differences in ability, personality, and interests; learning and motivation; psychological research methods; and the assessment and treatment of behavioral and affective disorders.
-
Knowledge of principles and processes for providing customer and personal services. This includes customer needs assessment, meeting quality standards for services, and evaluation of customer satisfaction.
-
