The Process
A Geriatric Care Manager will gather information, meet with the senior and loved ones able to attend in the home, and then provide a written report with recommendations. The assessment process with a Geriatric Care Manager will take about 2 hours.
The Report
In the People's Care assessment report information will be gathered into one document that you can use to plan care, and also take to other professionals such as the physician, attorney or financial planner. Areas included in the assessment report are: family and social history, health considerations and current medications, daily care needs, social and recreational needs, cognitive screening, home safety screening and behavioral considerations.
The Answer
With the assessment report and recommendations, the Geriatric Care Manager will help you discuss the results with your senior. If assistance is recommended and your senior loved one is resistant, we understand and can help. The Geriatric Care Manager is respectful of a desire for independence and honoring of life accomplishments. The goal of care is to provide assistance with health and safety as well as social engagement.
