Individual Service Plan
After a comprehensive assessment, the Geriatric Care Manager will create an Individual Service Plan for your loved one. This includes a

• Care Plan – identifies assistance with home making and personal care needs
• Engagement Plan – identifies social and recreational engagement activities
• Behavior Plan – identifies approaches for challenging behaviors, if any
• Success Plan – identifies how our success will be measured

Preparing and Supervising the Care Companion
The Geriatric Care Manager will choose the most appropriate Care Companion for your loved one based on skill and personal qualities. The Care Companion will be trained for your loved one's specific health, cognitive and social needs and will then implement the Individual Service Plan under the direction and supervision of the Geriatric Care Manager.

Physician Appointments
We will ensure your senior loved one is accompanied to appointments. The Geriatric Care Manager will attend all physician appointments with your senior loved one and then report the results to you upon request. We will assist your loved one by concisely preparing questions, collecting data the physician has requested, recording the physician's instructions and ensuring follow up.

Emergencies
The Geriatric Care Manager is your point of contact for all emergencies, 24/7. Should your senior loved one require an emergency room visit, the Geriatric Care Manager will be there as an advocate and to keep you informed.

Resources and Support
Now that your senior loved one is safe and well cared for, the Geriatric Care Manager is available to be your contact for resources and support.
