How it Works
The Geriatric Care Manager best understands your loved one's needs by completing a comprehensive geriatric assessment. She then meets with those interested in the senior's situation to review the recommendations, and provide appropriate referrals. The senior and family members decide on the course of action based on these recommendations.

If care services are to begin, the Geriatric Care Manager chooses the most appropriate Care Companion staff based on skill and personal qualities. The Geriatric Care Manager and our People's Care nurse prepare the Care Companion with written instructions and information specific to your loved one's physical, cognitive and social needs.

The Benefits
The Geriatric Care Manager is your point of contact for all questions regarding your senior loved one. She is simply one phone call away. It is the Geriatric Care Manager's responsibility to oversee your loved one's care, attend physician appointments and report the results to you, and be your resource and support. The Care Manager will have an answer to your questions or will do the research for you.
