Based on the above four principles, our Supported Living Service (SLS) is designed to assist adults with developmental disabilities, their family members, and the community at large, in utilizing behavioral strategies, interventions, and one-to-one assistance at home or in the community. This unique service enhances the client(s) and/or the family to survive and thrive in our society.

The primary goal of the program is to ensure that adults with developmental disabilities become as self sufficient as possible while receiving necessary support to live in their community. These support and services will be individualized to meet the self directed needs and desires of the individual.

• Exploring the living options (apartment, house, mobile home, yard, etc.)
• Assisting in locating the neighborhood that meets the needs of the individual
• Assisting the individual in finding affordable living options
• Assisting in applying for city, state and federal programs
• Ensuring the client expresses the appearance of their home
• Assisting the client find an appropriate roommate (if desired)
• Assisting the client in making their home safe

• Make everyday choices
• Take control of their lives in everyday situations
• To be the voice in their “self directed” services
• Understand what their needs and wants are and ensure it is followed
• Learn how a person communicates
• Choose appropriate support staff (hiring, job descriptions, etc.)
• Promote a safe and healthy lifestyle.
• Know their rights and responsibilities (tenant rights, labor law, etc.)
• Help to define their hopes and dreams.

• Create the bridge needed to connect clients with others in their community
• Learn the interests and hobbies of the client to assist them in meeting others
• Assist clients in learning to build conversations and interact with others
• Explore safe opportunities to meet others (church,social activities, etc.)
• Build the circle of support for the individual.

• Participation in the person centered planning process
• Reflect the needs and wants of the client through their plan
• Offering opportunities for clients to make choices, experience confidence and increase their self esteem
• Learning about the person so that we can tailor services to their wants and desires
• Communicating what the client wants
• Encouraging the person to do the things they are capable of doing
• Offering Personal Support Services to a client who needs assistance
• Responding to emergency situations (Staffing issues,safety, medical, etc.)

People's Care SLS is available 24 hours a day, on call basis. Our system of response to an emergency is based on the individual support plan from the ISP and is directed by the individual. The program will provide training in any area of information, procedures or devices needed to assist the consumer in feeling free from harm.

People's Care, SLS is available to clients who are developmentally disabled receiving services from the Regional Center and who desire support and services to live in their own home. Consumers do not need to have specific skills or criteria to participate in this program. The program will take clients that exhibit behaviors, have insulin diabetes, gastrostomies, tracheostomies, cancer (no hospice), and forensic issues.
